The underlying technology within Xchange was first incorporated into Ankota’s TherapEZ product, that enables home health agencies to pass referrals, share care plans, records and delivery schedules with contract therapy agencies.  Now the concept has been expanded to hospital discharge coordination with home care or skilled nursing, ACO primary care coordination with subcontracted home care services and coordination of care for children with special medical needs.  Xchange allows providers to refer a patient for a next level of treatment (such as post-acute care after hospital release) or subcontracted care (such as to a specialist), then the groups who are coordinating can share schedules, status, medical notes and more over the course of treatment.
