In Accountable Care: Organizing Provider Ecosystems, Ankota CEO Will Hicklen discusses the rapid organization of provider "ecosystems," with focus on the role that post-acute services plays to reduce avoidable readmissions and closely align with ACO-driven programs of care such as Care Transitions, Patient Centered Medical Home (PCMH), Community Based Care Management (CBCM), and more. Following predictable patterns that other networked industries have followed as they matured, "Ecosystems" of post acute providers will serve as a primary delivery model for care that is increasingly delivered outside of Hospitals. Topics covered will include steps providers can take immediately to position themselves to be a valued part of the ecosystem. The audience will include ACOs, Hospitals, post acute providers of all types, and primary care. 

The ACO Expo focuses on collaboration between ACOs and providers of post acute care. Hospitals across the country are implementing strategies intended to improve the quality of care processes within their facilities with varying degrees of success. However, one common need in most every health system is to improve the coordination of care between hospitals, primary care, and within the post acute continuum. This collaboration between hospitals, physicians and other post acute providers, along with the sharing of successful ideas and strategies, is the focus and purpose of the ACO Expo.
